More severe grades of reflux are manifest by increased dilatation of the ureter and renal pelvis as seen on the example at the right (grade III on a scale of I - VI). Notice that the bladder wall is not smooth; it is irregular. Sometimes reflux occurs as a result of increased pressure within the bladder caused by a hyperactive bladder muscle.
