Correct answer = D.
PEEPi (intrinsic PEEP, auto-PEEP, dynamic hyperinflation, breath-stacking) develops in patients with COPD when expiratory flow is diminished leading to incomplete exhalation of one breath before the next inspiration begins. Therefore PEEPi will be minimized by maneuvers which either decrease minute ventilation (i.e. RR or TV), improve expiratory flow (i.e. administration of bronchodilators), or increase the expiratory time (i.e. the inspiratory time by increasing inspiratory flow, eliminate inspiratory pause). PEEPi increases work of breathing by forcing the respiratory muscles to generate sufficiently negative pleural pressure on each breath to overcome the positive alveolar PEEPi before any airflow will occur. Application of extrinsic PEEP will offset the PEEPi and hence reduce the work of breathing. However, application of extrinsic PEEP will not reduce the PEEPi itself.
