Non-Loyola Student Research Information

April 2008

This packet contains an External Student Research Application Procedure sheet and the forms required to bring in an external summer temporary or volunteer student to work in a lab in your department.

The included forms are:

• External Student Research Application Procedure Form
• External Student Research Appointment Request Form
• I-9,Employment Eligibility Verification Form
• W-4
• Criminal Background Investigation Program of the Metropolitan Chicago Healthcare Council Form

Also required, but available departmentally is the Loyola University Health System - Employee Information Form (EIF) and the Loyola University of Chicago Employee Requisition.

It is highly recommended that the Administrator give the student the External Student Research Application Procedure form and then, upon completion, keep the packet of forms together and submit to Dr. Richard Kennedy, VP Health Sciences Research to begin the approval process.
