Order the Master Muscle List CD-ROM

The entire contents of the Master Muscle List web site are now available for your convenience on CD-ROM. The CD-ROM will enable you to view the Master Muscle List without establishing an internet connection. Please note that each CD-ROM comes with a single user license. All sales are final and no volume discounts are available at this time.

Requirements:

1. Computer (PC or Macintosh) with a CD-ROM drive.
2. PC: 486 or Pentium processor recommended running Windows 3.1 or higher
3. Macintosh: 68040 or PowerPC processor recommended running MacOS 7.0 or higher
4. WWW browser application (Netscape Navigator 3.0 / Internet Explorer 3.0 or higher recommended)

Instructions:

1. Print out and read the license agreement (retain a copy for your records)
2. Print out the order form (retain a copy of completed form for your records)
3. Fill out the order form below completely (note that placing an order implies acceptance of the license agreement)
4. Attach a check or money order for the appropriate amount made payable to Stritch School of Medicine - LUMEN Project (please do not send cash)
5. Mail the completed form and check to:
