Case #2:

An 18 year old white female comes to your office for a pre-college physical. Her history is basically non-contributory except she admits to beginning to smoke at age 16. She has continued to smoke as she feels that it makes her thinner. She denies alcohol and is sexually active without birth control. Her physical exam is unremarkable.
