How Do I Get a CD or Copies of My Films?

• Call (708) 216-1160 to request the CD or copies. (Please note there is a $5 charge per sheet of film, but CDs are free.)

• The wait is a minimum of 3 business days, although every effort will be made to accommodate emergency requests.

• Be specific regarding which records from what dates are needed.

• When your copies are ready for pick-up, go to the Lower Level of the Russo Pavilion, Room 0052 (Radiology File Room). You will need to complete a release form and show identification. If a family member picks up your copies, he/she will need a written authorization letter from you to release your film and will need to show identification.

• If you wish to have your copies mailed, please mail or fax a release form and we will send your copies by certified mail. Please allow us 3 business days to process your request and 7-10 days for the films to arrive.
