and Records
All
medical students must have a Medical History Report on file to use the
services.
These
forms are required for initial admission to the Stritch School of Medicine
and are important because they provide essential information about ongoing
health care and post treatment.
Failure
to supply this information makes a student ineligible to use the service.
All
students’ records are confidential.
Medical records are not made available to anyone without the
students’ permission.
