Address Change

• It is each student's responsibility to keep Registration & Records informed of his/her current contact info (local address, permanent address, phone, etc.) at all times.
• Please ensure that you keep your biographical records up-to-date with our office. You can check the contact information we have on file for you in myLUMEN. If it is not correct, please complete this form and return it to Registration & Records.
• Alternatively you may e-mail your changes or corrections to Julie Steinecker at jsteinecker@lumc.edu.
  
• Please remember to inform us promptly of any changes to your contact information. Our office periodically mails out important, time sensitive material to students. In addition, we provide mailing labels and address lists to many other offices within the school.

Name Change

• If your name changes during your time at Stritch, please complete this form and bring it to Registration & Records along with a certified copy of the legal documentation of your name change.
