Address or Name Change
- It is each student's responsibility to keep Registration & Records informed of his/her current contact info (local address, permanent address, phone, etc.) at all times.
- Please ensure that you keep your biographical records up-to-date with our office. If at any time in the future you need to update or correct your contact info (e.g., address, phone, etc), you may do so yourself through myLUMEN. When you log onto myLUMEN, click on the "Resources" tab at the top left and the bottom link down is "Update your Registration Information." Any changes or corrections can be entered directly onto that page (hit "submit update" at the bottom to save any new info you enter).
- Alternatively you may e-mail your changes or corrections to Julie Steinecker at email@example.com.
- Please remember to inform us promptly of any changes to your contact information. Our office periodically mails out important, time sensitive material to students. In addition, we provide mailing labels and address lists to many other offices within the school.
- If your name changes during your time at Stritch, please complete this form and bring it to Registration & Records along with a certified copy of the legal documentation of your name change.
