FLU VACCINE REQUIREMENT DETAILS:
All students, physicians and staff at Loyola University Medical Center are required to be vaccinated for the seasonal flu. Therefore, it is mandatory for all visiting students participating in electives at Loyola (and affiliated sites) to show proof of seasonal flu vaccination, beginning with the November elective rotations.
You will not be allowed to participate in your scheduled elective unless you are vaccinated and will be required to show proof of your seasonal flu vaccine before you will be allowed to begin your elective. Please provide the documentation of your vaccination to your elective coordinator upon arrival at your elective service.
Acceptable documentation includes a signed statement from the vaccine provider giving the date, type of vaccine, name and address of the provider, the manufacturer, lot number, and dose. If you have already had your flu vaccine this year but have misplaced your documentation of it, your elective coordinator will be able to give you a form to fill out attesting to this.
For more information on our visiting student electives, please contact Tess Kazda at tkazda@lumc.edu or 708-216-3222.
