    Admissions:  

___ Complete "Acknowledgment Form", which you will receive
       in a mailing from Admissions; sign and return via US mail.

___Complete "Transcript Inventory Form" which you will receive
       in a mailing from Admissions; sign and return via US mail.
___ Due 5/21/08

___Submit official transcript(s) from all undergraduate, graduate, or ___professional schools you have attended to the Office of--------- ___Admissions. (Due - 07/28/08)       

___Submit six (6) "Photographs" to the Office of Admissions.
___(Due- 07/07/08)

    Registration & Records:
 
___Locate and bring official "Proof of Citizenship"
       documents to Orientation in July.

___Complete Pre-registration form available on-line at  
       https://vault.lumc.edu/finaid/default.cfm .

    Financial Aid:

___Complete Financial Aid Checklist.

   Student Affairs:

___Complete the "Matriculating Student Questionnaire"

___Submit health and immunization information by
       completing both sides of the Medical History Report form.

___Waive out of, or purchase, Student Health Insurance.

___Complete and return "Roommate Wanted Listing Form"
       if applicable.

___Bring lock for personal locker at SSOM.

    Educational Affairs:

___Order textbooks for beginning courses in advance to
       insure receipt prior to start of classes (book sale
       sponsored by students will be held the first week of
       classes; availability of required books is not guaranteed).
