MANDATORY FLU VACCINATION

All LUMC, SSOM and Niehoff School of Nursing employees, volunteers and students will be required to receive an annual vaccination for the seasonal flu.  Gottlieb employees with direct patient contact must also receive a flu vaccination.  Any employee who does not get vaccinated by December 1, 2009 will not be able to work in order to prevent the spread of seasonal influenza to Loyola patients, visitors, employees and their families.  Absences related to lack of compliance with the mandatory seasonal influenza immunization program will be treated as unexcused and unpaid absences.

Dear Doctor:

The Loyola Occupational Health Service welcomes you to the Loyola University Health System!

In order to ensure your compliance with occupational health and safety recommendations, prior to assuming your house staff position with Loyola, you will need to provide documentation for your employee health record. Forms used at Loyola may be very similar to the documents you have on file at your medical school. Also, immunization information is likely most easily available through the student health record you were probably required to have in place in order to perform your student clerkships.

So that you can start your residency clinical rotations without delay, please return the following information by May 5, 2009 to:

  1. Loyola Evaluation History Form – Please complete. The information on this form will be used to generate an employee health record number and serve as the basis of your confidential employee health record.
     
  2. OSHA Respirator Medical Evaluation Questionnaire – Please complete. Answers to these nine questions will be used with your N95 fit testing done during Residency Orientation to qualify your respirator use at Loyola.
     
  3. Immunization records – Please request a copy of your immunization records from your medical school health office and send with your completed forms. To begin work, your employee health record must include proof of immunization or protective titers as follows:          

A.   Measles (protective titer or evidence of two immunizations after 12 months of age)

B.   Mumps (protective titer or evidence of two immunizations after 12 months of age)

C.  Rubella (protective titer or evidence of one immunization after 12 months of age)

D.  Varicella (protective titer, documentation of two doses of vaccine, or history of illness)

E.   Hepatitis B immunity (evidence of immunization and/or titer)

F.   Report of TB skin test applied and read during 90 days prior to assuming clinical responsibilities (or chest x-ray report done during 90 days prior to assuming clinical responsibilities).

Prior to assuming your housestaff position with Loyola, you are required to comply with several occupational/employee health requirements including:

    
completion of a medical/occupational questionnaire
    
compliance with infection control policies regarding
         immunizations
    
compliance with TB screening program, and
    
compliance with fit testing for an N95 respirator

In an effort to assure that you have ample time to comply with the medical center requirements, linked are the following:

Additionally, be advised that you are required to provide proof of the following:

immunity to varicella (protective titer, documentation of two doses of vaccine, or history of illness)

All forms and requisite documentation must be returned to the Department of Occupational/Employee Health Services by May 5, 2009.
