Continuing LOYOLA UNIVERSITY MEDICAL TRAINEES,
we are once again quickly approaching the start of a new academic year.
Each year, residents and fellows are required to complete an Annual Disclosure Statement and the Physician Annual Newsletter (Safety, Compliance, HIPAA). It is expected that these will be completed no later than April 15.
Both tools are located on the Loyola Portal System. The portal is accessible via the following URL: http://portal.luhs.org/template/dean/index.cfm. Log on the system by entering your ID and password in the appropriate area on the left hand navigation bar. Questions related to logging into the LUMC portal should be directed to the program administrative office or Help Desk (62160).
Annual Disclosure Statement
The Annual Disclosure Statement is among the links available once logged into the portal. The Annual Disclosure Statement provides opportunity to report your activities and relationships that may affect Loyola.
All LUMC residents and fellows are expected to complete this disclosure on the Loyola portal and
are encouraged to print out verification of completion. Verification should be submitted to your program coordinator for subsequent submission to the Office of Graduate Medical Education.

2009 Physician Annual
Newsletter (Safety, Compliance, HIPAA)
To complete the Annual Newsletter, select E-Learning, then click on Enroll in CBL. In the Display List by Curriculum area, scroll to the top and select
2009 Annual Mandatories. Click on Enroll to the left of
2009 Physician Annual Newsletter (safety, Compliance, HIPAA).
All LUMC residents and fellows are expected to comply with this training by reviewing the materials and completing the examination on the Loyola portal. You are
encouraged to print out the system transcripts to verify compliance. Verification should be submitted to your program coordinator for subsequent submission to the Office of Graduate Medical Education.
International Medical Graduates
Please click the following link to print your application for Continuation of your J-1 Visa Application. Submit all of your completed documents to your training program administration office no later than April 1 to ensure the processing of your application in a timely fashion and avoid a delay in your training.
