ECFMG Certificate

An unexpired ECFMG certificate must be provided. Although you must be ECFMG-certified by the program start date, you may complete your application documents before you begin your training program at Loyola. When you submit your documents, please include photocopies of your exam results. When you receive your ECFMG certificiate, you must provide a photocopy to your training program administrative office. Work Authorization/Visa

Appropriate current work authorization/visa documents are required for those who are in the united states on temporary or permanent work authorization.

Foreign National Information Form (FNIF)

Please click the following link for a Foreign National Information Form for you to fill out. This form must be completed and returned before you can begin training. All applicable questions must be answered. From your passport, a copy of both sides of your I-94 form (Arrival and Departure Record) and your U.S. Visa must be attached to this form when you return it.
