Student Health Services

Congratulations! Student Health Services (SHS) has been informed that you will be a member of the Loyola Stritch School of Medicine (SSOM) class of 2012. Prior to registration, you will need to comply with several SHS requirements.

Please complete the front page of the enclosed Medical History Report form.

In order to be in compliance with SSOM immunization policies, you must provide the following information:

  1. Blood test results documenting serologic immunity to measles (i.e. positive measles IgG). If not immune, you must include documentation of two vaccinations after 12 months of age.
  2. Blood test results documenting serologic immunity to mumps (i.e. positive mumps IgG). If not immune, you must include documentation of two vaccinations after 12 months of age.
  3. Blood test results documenting serologic immunity to rubella (i.e. positive rubella IgG). If not immune, you must include documentation of one vaccination after 12 months of age.
  4. Blood test results documenting serologic immunity to varicella (i.e. positive varicella IgG). If not immune, you must include documentation of two vaccinations, 4-8 weeks apart. History of varicella infection without documentation of positive immune titers does not satisfy this requirement.
  5. Blood test results documenting serologic immunity to Hepatitis B (i.e. positive Hepatitis B IgG). If not immune, you must include documentation of your initial three-shot vaccination series and all subsequent booster shots.
  6. Documentation of a negative Tuberculosis skin test (PPD) dated no earlier than April 1, 2008 or documentation of a negative chest x-ray dated no earlier than July 1, 2007 for those persons who are skin test positive.
  7. Documentation of previous Polio and Tetanus vaccinations, including a tetanus booster (Tdap) dated no earlier than July 1, 1998.

Vaccination records must be completed and signed by a health care provider (e.g., M.D., D.O., R.N., or other public health official). Vaccination dates must include Month, Day, and Year. Copies of original records may be submitted as proof of vaccination.

Vaccination records are not required in those cases in which the immune titer is positive.

A copy of all laboratory serology reports (both positive and negative) and a copy of your pertinent immunization records should be submitted to Student Health Services via mail, fax, or in person by July 14, 2008.

All Students who do not have their blood test results available can obtain these tests during orientation on a fee for service basis. Also, all Students who require additional vaccinations can have these administered during orientation on a fee for service basis.

Our staff will be in attendance during your orientation. We will meet with each of you to review your immunization records.You will not be allowed to begin classes until your Medical History Report form and immunization documentation are completed.
